Abstract

ABSTRACT. The goal of the article was to review Genesis II Total Knee System use according to AoA NJRR report. Genesis II has been widely used in Russian Federation since 1996. The AoA NJRR report was studied in order to learn connection between number of revision procedures and surgeon’s skills or type of the System, adequate choice among cemented, cementless and hybrid fixation, number of surgeries using Genesis II Total Knee System annually, average revision rate for Genesis II and early cumulative revision rate.

CONCLUSION. The systematic study of AoA NJRR allows us to make a conclusion that cemented fixation of Genesis II Total Knee System is mostly effective, and this fact is confirmed by domestic experience.

Abstract

AIM OF THE STUDY. The aim of the study is to analyze frequency and causes of dissection of the left main coronary artery and the right coronary artery dissection involving the aorta related to diagnostic and endovascular interventions as well as to describe modern approaches to treatment for these complications.

MATERIAL AND METHODS. The results of endovascular interventions in 2,317 patients were studied. Dissection of the left main coronary artery developed in 3 patients (0.13%), spreading to the aorta in 1 case.

RESULTS. coronary stent implantation closing the dissection of the left main coronary artery has been effective in 2 patients, and in 1 patient the dissection has not been immediately revealed.

CONCLUSIONS. A stent implanted to close the beginning of the dissection (“the entrance gate”) is the primary method for elimination of complications. conservative management of patients is possible if relatively limited dissection has no tendency to spread.

Abstract

INTRODUCTION. Upper extremity fractures comprise 84 % of all extremity fractures in children.

MATERIAL AND METHODS. In 2004–2013, 504 operations were performed in children aged 5 to 17 years for various injuries of the upper extremity using image intensifiers, in Minsk clinical centre of Trauma and orthopaedics of 6th Minsk clinical Hospital (children’s Department)

RESULTS. Immediate results were good or excellent in 98.8 % of patients. In 1.2 % of patients, post-traumatic transient neuropathy of the radial and median nerves was observed.

CONCLUSIONS. Minimally invasive operations with image intensifiers performed to treat injuries of the upper extremity in children are highly effective and allow open reduction and skeletal traction to be avoided.

Abstract

ABSTRACT. We report a case of progressive carcinoid heart disease (cHD) having caused readmission of a patient after excision of endocardium with arterial intima and multiple valve replacement. The data prove that in cases of combined valve heart disease of the right side it is necessary to assume the high probability of the carcinoid disease. Therefore, in spite of the high value of echocardiography, multispiral computed tomography (MScT), confirming the diagnosis, detecting metastases in different organs and determining the localization of the primary tumor, becomes particularly important. open heart surgery can be effective only when accompanied by operative therapy for the primary tumor with the use of somatostatin analogues.

Abstract

ABSTRACT. Myasthenia gravis is an autoimmune disease leading to weakness and pathological fatigue. The basis of the pathogenesis of myasthenia gravis is a disturbance of neurotransmission due to the influence of polyclonal autoantibodies on various structures of the neuromuscular junction. The recurrent or progressive course is common for the disease. It is extremely difficult to suspect and diagnose this pathology for an anesthesiologist-resuscitator who doesn’t treat patients with myasthenia gravis in everyday practice, since the patients often arrive to mainstream hospitals in critical condition with incorrect diagnosis. only specialized units of neurology have electromyographs and equipment for particular immunological probes. We report a case of myasthenia gravis in a patient admitted to a hospital emergency room with the other disease suspected.

Abstract

ABSTRACT. Acute cerebrovascular accident remains one of the most important issues of modern clinical neurology and holds a leading place among the causes of disability and mortality. The cerebral stroke has not only medical, but also social and economic effects, as it accounts for disability in population. According to the National Register, 31% of stroke patients require constant care, 20 % of patients cannot walk unassisted and only 20% of patients return to their previous work. New approaches to the treatment for ischemic stroke include the use of modern high-performance reperfusion of the brain during the first hours of the disease. The case of reperfusion therapy in a patient, performed twice with an interval of 8 months in Prof. S.V. Ochapovskiy Regional clinical Hospital No. 1 of the Ministry of Health of the Krasnodar Territory is reported.

Abstract

ABSTRACT. The article reviews the history of neuropsychological rehabilitation. It begins with the description of first rehabilitation programs developed by Paul Broca and Shepherd Franz. Franz’s experimental work for motor recovery in monkeys and correlation between active movement or affected limb immobilization and rehabilitation outcomes are described in further details. Special focus is given on ideas of famous German neurologist and psychiatrist Kurt Goldstein, who laid the foundation for modern approach in rehabilitation. Goldstein developed the idea of connection between rehabilitation and patient’s daily life. He also pointed out the necessity of psychological care of patients with brain damage.

Russian neuropsychological approach is presented by its founders L.S. Vygotskiy and A.R. Luriya. Aspects of higher mental processes structure and options of its correction such as “cognitive prosthesis” are described in the sense of the approach.

Y. Ben-Yishay, G. Prigatano, B. Wilson represent neuropsychological rehabilitation of the second half of the 20th century. The idea of a holistic approach for rehabilitation consists of such important principles as patient’s active involvement in a process of rehabilitation, work of a special team of rehabilitation professionals, inclusion of patient’s family members. The short review of a new rehabilitation approach for patients in coma, vegetative states and critical patients under resuscitation is given.

Abstract

ABSTRACT. The history of the N.V. Sklifosovsky Research Institute for Emergency Medicine has been studied to divide it into periods and characterize them. It was shown that the stages of the Museum existence coincide with the periods of the Institute history. So called “Period of Yudin” can be identified separately. The great contribution of the Institute’s Surgeon-in-chief prof. S. S. Yudin to the foundation of the Museum is emphasized, as well as transformation of the museum of the history of surgery with its concept and collections into the museum of the multidisciplinary medical institution, which represents the background and achievements of the Institute.
