В патогенезе сепсиса значимая роль принадлежит бактериальному эндотоксину (ЛПС - липополисахарид).</p><p>Цель Оценка эффективности использования селективной гемосорбции липополисахаридов (СГЛ) в комплексном лечении сепсиса.</p><p>Материал и методы Обследованы 65 пациентов, у которых развился сепсис с подозрением или наличием грамотрицательной инфекции. Пациенты ретроспективно были разделены на две группы. В одной 27 пациентам применяли гемосорбент, содержащий Полимиксин-В (РМХ) с использованием колонок Тореймиксин, в другой 38 больным гемосорбент Альтеко (ЛПС-А).</p><p>Результаты Установлено, что 28-дневная летальность составила 11,1% в группе PMX и 28,9% — в группе ЛПСА, р=0,091, 60-дневная - 33,3% и 55,3% соответственно (р=0,065). Применение СГЛ способствовало снижению активности эндотоксина (ЕАА) с 0,52 (0,39; 0,65) до 0,40 (0,36; 0,57); EU (р=0,330) в группе PMX и с 0,59 (0,42; 0,72) до 0,54 (0,40; 0,81); EU (р=0,981) в группе ЛПС-А. При этом уровень прокальцитонина (ПКТ) в крови статистически значимо снижался с 8,4 (3,6; 29,0) до 4,8 (1,9; 36,3) нг/мл (р=0,0117) только в группе ЛПС-А. Уровень С-реактивного белка (СРБ) в крови статистически значимо снижался только в группе РМХ — с 205 (154; 264) до 162 (106; 202) мг/л (р&lt;0,001). После процедур СГЛ отмечалась тенденция к снижению уровня в крови цитокинов в обеих группах.</p><sec><title>Выводы 1</title><p>Выводы 1. При использовании колонок с Полимиксином-В в процессе гемоперфузии отмечается тенденция к лучшей выживаемости пациентов по сравнению с результатами адсорбции липополисахарида с использованием колонок Альтеко: так, 28-дневная летальность составила 11,1 и 28,9% (различия статистически незначимы).</p></sec><sec><title>2</title><p>2. В результате процедуры селективной гемосорбции липополисахаридов на гемосорбенте с Полимиксином-В в крови статистически значимо снижался уровень С-реактивного белка (на 21%), отмечалось статистически незначимое снижение уровня активности эндотоксина (на 23,1%), содержания в крови липополисахарид-связывающего белка (на 21,6%), прокальцитонина (в 2,4 раза), пресепсина (на 20%), а также уровня интерлейкина-6 (в 3,4 раза) и интерлейкина10 (в 1,6 раза). Адсорбция липополисахарида с использованием колонок Альтеко ведет к статистически значимому снижению уровня в крови прокальцитонина (в 1,8 раза), статистически незначимо снижались уровни в крови: активности эндотоксина (на 9,3%), содержания в крови липополисахарид-связывающего белка (на 28,6%), интерлейкина-6 (в 3,8 раза), интерлейкина-10 (в 7,1 раза) и растворимого рецептора к интерлейкину-2 (в 2,2 раза).</p></sec></abstract><trans-abstract xml:lang="en"><p>Background Sepsis and septic shock are formidable and complications in surgery with mortality 20–50%. In the pathogenesis of sepsis, a significant role belongs to bacterial endotoxin (LPS - liposaccharide).</p><p>Aim of study Assessment of the efficacy of selective lipopolysaccharides hemosorption (SLH) in treatment of sepsis.</p><p>Material and methods We examined 65 patients with developed sepsis or suspected presence of gram-negative infection. Patients were retrospectively divided into two groups. In Group 1, 27 patients received Polymyxin B hemoperfusion using Toraymyxin cartridges. In Group 2 (38 patients), adsorber Alteco (LPSA) was used.</p><p>Results It was established that 28-day mortality was 11.1% in Polymyxin group and 28.9% in LPS group A, p = 0.091, 60-day mortality was 33.3 and 55.3%, respectively (p=0.065). The use of SLH contributed to a decrease in the activity of endotoxin (EAA) from 0.52 (0.39; 0.65) to 0.40 (0.36; 0.57) EU (p=0.330) in Polymyxin group and from 0.59 ( 0.42; 0.72) to 0.54 (0.40; 0.81) EU ( p = 0.981) in the LPS-A group. At the same time, the level of procalcitonin (PCT) in the blood statistically significantly decreased from 8.4 (3.6; 29.0) to 4.8 (1.9; 36.3) ng/ml (p=0.0117) only in the LPS-A group. The level of C-reactive protein (CRB) in the blood statistically significantly decreased only in the Polymyxin group, from 205 (154; 264) to 162 (106; 202) mg/L (p&lt;0.001). After SPH procedures, there was a tendency to a decrease in the level of blood cytokines in both groups. </p><sec><title>Conclusion 1</title><p>Conclusion 1. The trend of better survival among patients was noted during hemoperfusion when using Polymyxin B in comparison with the results of adsorption of lipopolysaccharide with Alteco cartridges: so, 28-day mortality was 11.1 and 28.9%, respectively (statistically not significant).</p></sec><sec><title>2</title><p>2. As a result, the procedure of selective lipopolysaccharides hemosorbtion on hemosorbents with Polymyxin B in blood significantly decreased level of C-reactive protein (21%), there was statistically insignificant decrease in the level of endotoxin activity (23.1%), lipopolysaccharide binding protein (21.6%), procalcitonin (2.4 times), presepsin (20%), as well as the level of interleukin-6 (3.4 times) and interleukin-10 (1.6 times) .
