Высокий риск развития венозных тромбозов у пострадавших с сочетанной травмой связан с рядом предрасполагающих факторов, таких как прямое повреждение сосудов, длительная иммобилизация, значительные изменения в системе гемостаза, а также возможное оперативное вмешательство.</p></sec><sec><title>ЦЕЛЬ РАБОТЫ</title><p>ЦЕЛЬ РАБОТЫ. Оценка частоты, эхосемиотики и эволюции венозных тромбозов, развившихся на фоне различной медикаментозной профилактики у пострадавших с политравмой.</p></sec><sec><title>МЕТОДЫ</title><p>МЕТОДЫ. Проанализированы результаты УЗИ вен нижних конечностей в динамике у 610 пострадавших с различной медикаментозной профилактикой. Антиагреганты получали 314 пациентов 1-й группы, низкомолекулярные гепарины — 186 больных второй и оральные антикоагулянты — 110 больных 3-й группы.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. Развившиеся тромбозы различались по частоте (52,5% — в 1-й, 15,6% — во 2-й и 10% — в 3-й группе), распространенности, характеру проксимальной границы, с увеличением доли неокклюзионных поражений во 2-й и 3-й группах, срокам и степени реканализации.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. При использовании низкомолекулярных гепаринов (НМГ) и оральных антикоагулянтов (ОАК) риск развития венозных тромбозов снижается в 3,3 раза и 5,2 раза соответственно по сравнению с группой больных, получавших в раннем посттравматическом периоде только антиагреганты (ААГ), а реканализация начинается на 1−2 нед раньше и с более эффективным восстановлением просвета. На фоне применения современных антикоагулянтов уменьшается частота венозных тромбозов, но в их структуре возрастает доля неокклюзионных, в том числе флотирующих, что диктует необходимость УЗ-наблюдения. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title> INTRODUCTION</title><p> INTRODUCTION. High risk of venous thrombosis in patients with multisystem trauma is associated with a number of precipitating factors, such as direct damage to vessels, prolonged immobilization, major changes in the hemostatic system, as well as possible surgical intervention.</p></sec><sec><title>THE PURPOSE OF THE STUDY</title><p>THE PURPOSE OF THE STUDY. Analysis of the incidence, echosemiotics, and evolutionof venous thrombosis in the course of various prophylactic drug therapies in patients with polytrauma.</p></sec><sec><title>METHODS</title><p>METHODS. The results of leg veins ultrasonography in 610 patients with various prophylactic drugtherapies have been analyzed over time. Antiplatelet agentswere received by 314 patients of the first group, low molecular weight heparins — 186 patients of the second group, oral anticoagulants — 110 patients of the third group. THE RESulTS Evolved thromboses varied in frequency (52.5% in the first group, 15.6% in the 2nd group and 10% in the 3rd group), prevalence, nature of the proximal border, with an increase in the proportion of nonocclusive lesions in the 2nd and 3rd groups, as well as in start time and degree of recanalization.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION. Low molecular weight heparins and oral anticoagulants reduce the risk of venous thrombosis by 3.3 and 5 times respectively, compared to the group of patients who did not receive anticoagulants in the early posttraumatic period; recanalization begins 1−2 weeks earlier with more effective restoration of the lumen.
