Одной из актуальных проблем среди пациентов старшего возраста на сегодняшний день является дегенеративный стеноз аортального клапана (АК). Консервативное лечение симптомов хронической сердечной недостаточности при стенозе АК улучшает состояние больного лишь на время, тогда как хирургическое лечение в виде замены несостоятельного АК признано основным эффективным методом лечения порока. В последнее время разработаны альтернативные технологии протезирования АК, направленные на снижение неблагоприятного влияния искусственного кровообращения (ИК) на пациентов высокого риска и минимизацию объема оперативного вмешательства.</p><p>Цель исследования Оценить непосредственные результаты хирургического лечения аортального стеноза с использованием различных методов у пациентов старше 70 лет.</p><p>Материал и методы В статье представлены результаты лечения 64 пациентов старше 70 лет с изолированным стенозом АК, оперированных по различным хирургическим методикам с июля 2016 г. по январь 2018 г. Все пациенты были разделены на три группы, различающиеся по тяжести исходного состояния пациентов и методу лечения. Группу 1 (транскатетерная имплантация протеза АК, EuroSCORE II — 21,81%) составили 19 пациентов, группу 2 (бесшовная имплантация протеза Perceval в условиях ИК, EuroSCORE II — 13,81%) — 13 пациентов и группу 3 («стандартное» протезирование АК, EuroSCORE II — 9,89%) — 32 пациента.</p><p>Результаты В 1-й группе умерли 2 пациента, госпитальная летальность составила 10,5%. Во 2-й и 3-й  группах умерли по одному пациенту, госпитальная летальность составила 7,6% и 3,1% соответственно. Трем пациентам (15,7%) из группы TAVI после установки протезов Medtronic Core Valve и двум пациентам (15,3%) из группы Perceval потребовалась имплантация постоянного электрокардиостимулятора.</p><p>Заключение Полученные результаты протезирования АК по различным методикам позволили расширить показания к коррекции стеноза АК у пациентов старшей возрастной группы с высоким хирургическим риском операции в условиях ИК, которые ранее не рассматривались кандидатами на хирургическое лечение аортального порока из-за возраста и тяжести сопутствующей патологии.</p></abstract><trans-abstract xml:lang="en"><p>Background As life expectancy and quality of health improve, more and more people reach old age, and so does the number of heart diseases. One of the most urgent problems among elderly patients is degenerative stenosis of the aortic valve (AV). The conservative treatment of symptoms of chronic heart failure with AV stenosis improves the patient’s condition only for a while, whereas surgical treatment such as replacement of AV is recognized as the main effective method of treating a defect. Recently, alternative technologies for prosthetic AV have been developed, aimed at reducing adverse effects of artificial circulation (AC) in high-risk patients and minimizing the scope of surgical intervention.</p><p>Aim of study The aim of the study was to evaluate the immediate results of surgical treatment of aortic stenosis using different methods in patients over 70.</p><p>Material and methods The article presents the results of treatment of 64 patients over 70 with isolated AV stenosis, operated with different surgical techniques from July, 2016 to January, 2018. All patients were divided into three groups, differing in the severity of the initial condition and the method treatment. Group 1 (transcatheter implantation of the prosthetic AV, EuroSCORE II — 21.81%) consisted of 19 patients, Group 2 (non-suture implantation of a Perceval prosthetic valce under the AC, EuroSCORE II — 13.81%) consisted of 13 patients and Group 3 (“standard” prosthetics, EuroSCORE II — 9.89%) consisted of 32 patients.</p><p>Results In Group 1, two patients died, the hospital mortality was 10.5%. In Group 2 and Group 3, one patient died, the hospital mortality was 7.6 and 3.1%, respectively.
