Повреждения заднего отдела стопы при сочетанной травме встречаются в 2,5 раза чаще, чем у пациентов с изолированными повреждениями. Травма стопы нередко не диагностируется на реанимационном этапе лечения. До 22% пострадавших, перенесших повреждения стоп, впоследствии становятся инвалидами.</p></sec><sec><title>Цель</title><p>Цель. Определить особенности травматических повреждений заднего отдела стопы у пациентов с сочетанной травмой.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Проведен ретроспективный анализ 89 пациентов с повреждениями заднего отдела стопы, поступивших в НИИ СП им. Н.В. Склифосовского в 2011–2013 гг.</p></sec><sec><title>Результаты</title><p>Результаты. Выявлены 128 переломов пяточных и таранных костей. Преобладали мужчины среднего возраста (39±14,1 года).</p></sec><sec><title>Заключение</title><p>Заключение. Выявлены следующие особенности: основной механизм травмы — кататравма (64%). В составе сочетанной травмы имеется до 40% пациентов с повреждением более двух анатомических областей тела. Отмечается частое сочетание с травмой нижнегрудного и поясничного отделов позвоночника (29%). Для 38% пострадавших характерно наличие полисегментарных перело­мов нижних конечностей. У пациентов с политравмой высокая частота множественных повреждений стоп (35%), особенно билатеральных переломов пяточных костей. До 60% открытых переломов III типа по Gustilo–Anderson.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Rearfoot injuries occur 2.5 times more often in a multisystem trauma than in patients with an isolated injury. The foot trauma is often misdiagnosed at the stage of intensive care treatment. Up to 22% of victims with foot injuries become disabled.</p></sec><sec><title>Aim of the study</title><p>Aim of the study. The aim of the study is to determine distinctive characteristics of rearfoot injuries in patients with multisystem trauma.</p></sec><sec><title>Material and methods</title><p>Material and methods. We peformed a retrospective study of 89 cases of rearfoot injuries in patients admitted to the N.V. Sklifosovsky Research Institute for Emergency Medicine in 2011–2013.</p></sec><sec><title>Results</title><p>Results. There were 128 calcaneus and talus fractures. Middle-aged male patients (39±14.5 years) prevailed.</p></sec><sec><title>Conclusion</title><p>Conclusion. We found several peculiarities: the common mechanism of an injury was the fall from a height (64%). Up to 40% of patients with multisystem trauma had injuries of two and more anatomic areas. The multisystem spinal (lower thoracic and lumbar) injury was detected in 29% of patients. In 38% of cases we revealed polysegmental fractures of lower limbs. In patients with polytrauma, multiple foot injuries were diagnosed in 31 cases (35%), most common were bilateral calcaneus fractures.
