Нетравматическое субарахноидальное кровоизлияние (САК) вследствие разрыва церебральных аневризм остаётся одной из наиболее тяжёлых форм острой цереброваскулярной патологии с высоким риском отсроченной ишемии головного мозга. Одним из ключевых патогенетических механизмов при данной патологии являются выраженные нарушения эндогенной регуляции сосудистого тонуса и развитие окислительного стресса. Ингаляционная терапия оксидом азота рассматривается как потенциальный метод модуляции сосудистого тонуса и снижения риска сосудистого спазма.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Оценить влияние ингаляционной терапии оксида азота на выраженность окислительного стресса и нарушения факторов эндогенной регуляции сосудистого тонуса у пациентов в остром периоде нетравматического САК вследствие разрыва церебральных аневризм.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. В исследование включены 57 пациентов в остром периоде САК, госпитализированные в НИИ СП им. Н.В. Склифосовского в период 2020–2025 годов. Основную группу составили 25 пациентов, получавших ингаляционную терапию оксидом азота (50–80 ppm, в течение 3–24 ч/сутки, 4–7 суток после операции); группу сравнения — 32 пациента без данной терапии. Критерии включения: массивное базальное кровоизлияние, клипирование аневризмы в первые 72 часа, тяжесть состояния II–IV по Hunt–Hess. Оценивали уровни малонового диальдегида (МДА), общей антиоксидантной активности, стабильных метаболитов оксида азота (NOx), ангиотензинпревращающего фермента (АПФ), глюкозы, лактатдегидрогеназы (ЛДГ) в сыворотке крови и ликворе. Статистический анализ проводили с использованием U-критерия Манна–Уитни, уровень значимости — p&lt;0,05.</p></sec><sec><title>Результаты</title><p>Результаты. У пациентов, получавших терапию, уровень NOx в сыворотке статистически значимо повышался во все сроки наблюдения (p&lt;0,05). Коэффициент NOx/АПФ был выше по сравнению с группой без терапии и приближался к значениям контрольной группы, что свидетельствовало о восстановлении баланса между вазодилатирующими и вазоконстрикторными механизмами. Уровень МДА и коэффициент окислительного стресса были повышены в обеих группах, но достоверных различий между ними не выявлено. Уровень ЛДГ и глюкозы в сыворотке и ликворе повышался в обеих группах, различия между ними статистически незначимы.</p></sec><sec><title>Заключение</title><p>Заключение. Ингаляционная терапия оксидом азота способствует восстановлению баланса факторов сосудистой регуляции без усиления окислительного стресса и ишемических повреждений. Метод может рассматриваться как дополнительный компонент комплексной терапии, однако требует дальнейшего изучения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Non-traumatic subarachnoid hemorrhage (SAH) due to ruptured cerebral aneurysms remains one of the most severe forms of acute cerebrovascular disease with a high risk of delayed cerebral ischemia. One of the key pathogenetic mechanisms in this pathology is severe impairment of endogenous regulation of vascular tone and the development of oxidative stress. Inhaled nitric oxide therapy is considered a potential method for modulating vascular tone and reducing the risk of vasospasm.</p></sec><sec><title>The aim of the study</title><p>The aim of the study. Was to evaluate the effect of inhaled nitric oxide therapy on the severity of oxidative stress and endogenous factors involved in regulation of vascular tone in patients with acute non-traumatic SAH due to ruptured cerebral aneurysms.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study included 57 patients with acute SAH hospitalized at the N.V. Sklifosovsky Research Institute for Emergency Medicine between 2020 and 2025. The study group consisted of 25 patients receiving inhaled nitric oxide therapy (50–80 ppm, 3–24 hours per day, 4–7 days postoperatively); the comparison group consisted of 32 patients without this therapy. Inclusion criteria: massive basal hemorrhage, aneurysm clipping within the first 72 hours, and Hunt-Hess stage II–IV. Levels of malondialdehyde (MDA), total antioxidant activity, stable nitric oxide (NOx) metabolites, angiotensin-converting enzyme (ACE), glucose, and lactate dehydrogenase (LDH) were assessed in serum and cerebrospinal fluid. Statistical analysis was performed using the Mann-Whitney U test, with a significance level of p&lt;0.05.</p></sec><sec><title>Results</title><p>Results. In patients receiving therapy, serum NOx levels increased statistically significantly at all observation periods (p&lt;0.05). The NOx/ACE ratio was higher than in the untreated group and approached the control group’s values, indicating restoration of the balance between vasodilatory and vasoconstrictive mechanisms. MDA levels and the oxidative stress index were elevated in both groups, but no significant differences were found. Serum and cerebrospinal fluid LDH and glucose levels increased in both groups, with statistically insignificant differences.</p></sec><sec><title>Conclusions</title><p>Conclusions. Inhaled nitric oxide therapy helps restore the balance of endogenous factors involved in regulation of vascular tone without increasing oxidative stress and ischemic damage.
