На долю спаечной кишечной непроходимости приходится до 2,4% всех лапаротомий. Выбор тактики ведения пациента напрямую зависит от того, имеются ли признаки (в том числе и компьютерно-томографические (КТ)-признаки) наличия странгуляции кишечника и (или) ишемии кишечной стенки.</p></sec><sec><title>Цель</title><p>Цель. Выявление КТ-признаков, достоверно связанных с ишемическими изменениями кишечника, а также КТ-предикторов необратимых изменений кишечника, которые могут потребовать проведения резекции во время операции.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Материалом для исследования послужили ретроспективно оцененные 53 КТ-исследования органов брюшной полости с внутривенным контрастным усилением, выполненные на этапе поступления у пациентов, прооперированных по поводу спаечной кишечной непроходимости. Оценивали следующие признаки: наличие симптома «клюва» в переходных зонах, толщину кишечной стенки, снижение контрастирования кишечной стенки, локальный отёк мезентериальной клетчатки, наличие «замкнутой» петлевой непроходимости, признак «плотного» кишечного содержимого в тонкой кишке, симптома «жировой выемки».</p></sec><sec><title>Результаты</title><p>Результаты. В проведённом исследовании было доказано, что утолщённая кишечная стенка, чётко выраженный симптом «клюва» в двух соседних точках, наличие замкнутой петлевой непроходимости, плохое контрастирование стенки кишки и каловое содержимое в тонкой кишке являются независимыми предикторами проведения резекции кишки. На основе полученных результатов был предложен алгоритм визуальной оценки кишечника на КТ органов брюшной полости у больных со спаечной кишечной непроходимостью.</p></sec><sec><title>Заключение</title><p>Заключение. Используя метод компьютерной томографии с внутривенным контрастным усилением, а также визуальный анализ полученных томограмм по предложенному алгоритму, возможно обнаружить признаки странгуляции при спаечной кишечной непроходимости с чувствительностью 69%, специфичностью 97% и точностью до 88%.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Adhesive intestinal obstruction is responsible for up to 2.4% of all laparotomies. The choice of patient management strategy directly depends on whether there are indications (including CT findings) of intestinal obstruction and/or ischemia of the intestinal wall.</p></sec><sec><title>The Aim of Our Study</title><p>The Aim of Our Study. Identification of CT signs that are reliably associated with ischemic intestinal changes, and CT predictors of irreversible intestinal changes that may require surgical resection.</p></sec><sec><title>Material and Methods</title><p>Material and Methods. The study used 53 retrospective CT scans of the abdomen with intravenous contrast enhancement, performed at the time of admission on patients undergoing surgery for adhesive obstruction. These scans were evaluated for the following signs: the presence of the “beak” sign in the transition zones, the thickness of the intestinal wall, a decrease in contrast of the intestinal wall, local edema of mesenteric tissue, the presence of “closed-loop” obstruction, a small-bowel faeces sign, and the “fat-notch” sign.</p></sec><sec><title>Results</title><p>Results. The study found that a thickened intestinal wall, a prominent “beak” at two adjacent points, the presence of a “closed loop” obstruction, and poor contrast between the intestinal wall and a small-bowel faeces sign were all independent predictors of the need for intestinal resection. Based on the obtained results, an algorithm for visual assessment of the intestine on CT of abdominal organs in patients with adhesive intestinal obstruction was proposed.</p></sec><sec><title>Conclusion</title><p>Conclusion.
