Тотальное эндопротезирование коленного сустава (ТЭКС) включает замену всех отделов коленного сустава для восстановления работоспособности и функции. ТЭКС — один из самых распространённых и надёжных хирургических методов лечения заболеваний коленного сустава. Однако некоторым пациентам требуется ревизионное эндопротезирование коленного сустава (РТЭКС) после ТЭКС по разным причинам, включая механический износ, расшатывание имплантата, проседание, инфекцию, нестабильность, перипротезный перелом, тугоподвижность. К сожалению, общий результат РТЭКС не столь удовлетворительный, как для первичного ТЭКС из-за многих факторов достигнутого успеха и факторов риска. Для улучшения результатов РТЭКС мы используем, в том числе применение связанных ревизионных эндопротезов с вращающимся шарниром, включая лечение пациентов с потерей костной ткани в метафизарном отделе коленного сустава. При предоперационном планировании РТЭКС мы используем классификацию костных дефектов, варианты восстановления суставной линии, что позволяет сделать индивидуальный подбор типа (вида) и компонентов модульного ревизионного эндопротеза. Клинический пример также позволяет обосновать выбор типа (вида) эндопротеза, а именно ревизионного эндопротеза с вращающимся шарниром.</p></abstract><trans-abstract xml:lang="en"><p>This article discusses some of the considerations in planning a revision total knee arthroplasty (RTKA). Total knee arthroplasty (TKA) involves replacing all parts of the knee joint to restore functionality and function. TKA is one of the most common and reliable surgical treatments for knee disorders. However, some patients require revision total knee arthroplasty (RTKA) after TKA for a variety of reasons, including mechanical wear, implant loosening, subsidence, infection, instability, periprosthetic fracture, and stiffness. Unfortunately, the overall outcome of RTKA is not as satisfactory as that of primary TKA due to many factors, including success rates and risk factors. Our approach to improving RTKA outcomes includes the use of linked, rotating-hinge revision prostheses, as well as the treatment of patients with metaphyseal bone loss. In preoperative planning of RTKA, we use the classification of bone defects, options for restoring the joint line, which allows for individual selection of the type (kind) and components of a modular revision endoprosthesis.
