Свернувшийся гемоторакс — наиболее частое осложнение проникающих ранений груди, требующее хирургического лечения у большинства пациентов.</p></sec><sec><title>МАТЕРИАЛ И МЕТОДЫ</title><p>МАТЕРИАЛ И МЕТОДЫ. За период с 2002 по 2012 г. видеоторакоскопия была выполнена 51 пострадавшему с осложнениями лечения проникающего ранения груди. Свернувшийся гемоторакс был выявлен у 27 пострадавших (52,9%).</p></sec><sec><title>РЕЗУЛЬТАТ</title><p>РЕЗУЛЬТАТ. Установлено, что основной причиной развития этого осложнения являлась неадекватная функция плеврального дренажа. Свернувшийся гемоторакс развился после дренирования плевральной полости и первичной хирургической обработки ран груди у 12 пострадавших (44,4%), после атипичной торакотомии — у 8 пациентов (29,6%) и у 7 пострадавших (25,9%) после типичной торакотомии. Средний интервал между операциями был 8,1±5,0 сут.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Наилучшие результаты лечения свернувшегося гемоторакса были достигнуты при раннем его выявлении в случаях торакоскопической эвакуации в диапазоне от 3 до 7 сут (4,7±2,1). Выполнение видеоторакоскопии в сроки позже 7 сут сопряжено с возрастанием объема хирургического вмешательства, что существенно увеличивает продолжительность лечения пострадавших. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND. Clotted hemothorax is the most common complication of the chest injury requiring surgical treatment in most patients.</p></sec><sec><title>MATERIAL AND METHODS</title><p>MATERIAL AND METHODS. Videothoracoscopy was performed in 51 patients with complications of penetrating chest trauma in 2011–2012. Clotted hemothorax occured in 27 cases (52.9%).</p></sec><sec><title>RESULTS</title><p>RESULTS. It was found that the main cause of this complication was inadequate pleural drainage effect. Clotted hemothorax developed after drainage of the pleural cavity and primary surgical debridement in 12 patients (44.4%), in 8 patients (29.6%) after atypical thoracotomy and in 7 patients (25.9%) after typical thoracotomy. The average interval between operations was 8.1±5.0 days.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION. The best results of treatment for clotted hemothorax were achieved under the early detection of clotted hemothorax in case of thoracoscopic evacuation in the range from 3 to 7 days (4.7±2.1).
