ЦЕЛЬ ИССЛЕДОВАНИЯ. Оценить эффективность лечения острого панкреатита путем применения раннего энтерального питания и селективного введения питательных смесей в начальные отделы тощей кишки, минуя двенадцатиперстную кишку.

МАТЕРИАЛ И МЕТОДЫ

МАТЕРИАЛ И МЕТОДЫ. Разработанный метод комплексного лечения острого панкреатита на основе использования панкреатодуоденального катетера. Принцип метода заключается в профилактике развития синдрома кишечной недостаточности путем комбинации параметров, направленных на изоляцию двенадцатиперстной кишки, и предотвращении поступления в нее любых жидких сред и обеспечения селективного введения глюкозо-солевых растворов и (или) энтеральных питательных смесей непосредственно в начальные отделы тощей кишки. В настоящее время в нескольких российских клиниках проводят научно-исследовательскую работу, план которой одобрен локальными этическими комитетами клиник. В данной статье приводятся промежуточные результаты анализа полученных данных. В работу включались среднетяжелые и тяжелые формы (согласно классификации Атланта, 2012) острого билиарного, без показаний для проведения эндоскопической ретроградной холангиопанкреатографии, и алиментарно-алкогольного панкреатита. Первую группу составили больные, получавшие исключительно стандартную терапию согласно клиническим рекомендациям российского общества хирургов 2020 года. Вторая группа больных получала стандартную терапию в комбинации с применением нового метода селективного энтреального питания. Период наблюдения продолжался до окончания стационарного лечения и включал 5 обязательных визитов и заключительный визит к врачу. Через 3 месяца и более после выписки из госпиталя проводился опрос пациентов с целью выявления отсроченных эпизодов развития осложнений острого панкреатита. При этом оценивали частоту инфекционных осложнений и хирургических вмешательств.

РЕЗУЛЬТАТЫ

РЕЗУЛЬТАТЫ. В исследование включены 148 больных. Все пациенты были равномерно распределены по этиологии, полу, возрасту и масса-ростовому индексу. В результате проведенного анализа было установлено, что количество пациентов с инфекционными осложнениями снизилось с 19,3% в группе сравнения до 6,67% в исследуемой группе (р=0,041). Количество пациентов, которым были выполнены хирургические операции, в исследуемой группе составило 5,0%, а в группе сравнения — 19,3% (р=0,017).

ЗАКЛЮЧЕНИЕ

ЗАКЛЮЧЕНИЕ. Результат проведенного клинического исследования показал, что новый метод в комплексном лечении острого панкреатита и раннее энтеральное кормление в I фазу заболевания положительно влияет на течение, уменьшая частоту инфицированного панкреонекроза и количество проводимых хирургических манипуляций.

AIM OF STUDY

AIM OF STUDY. The study was aimed to evaluate the effectiveness of treating acute pancreatitis by early enteral nutrition and selective introduction of nutritional mixtures directly into the initial sections of the jejunum, bypassing the duodenum.

MATERIAL AND METHODS

MATERIAL AND METHODS. A comprehensive treatment method for acute pancreatitis was developed based on the use of a pancreatoduodenal catheter. The method focuses on preventing intestinal failure syndrome by isolating the duodenum and preventing any liquid from entering it while ensuring the selective introduction of glucose-saline solutions and/or enteral nutritional mixtures directly into the initial sections of the jejunum. Several Russian clinics are currently conducting the research approved by local ethics committees. This article presents preliminary results from the data analysis. The study included moderate to severe forms (according to the 2012 Atlanta classification) of acute biliary pancreatitis without indications for endoscopic retrograde cholangiopancreatography, as well as alimentary-alcoholic pancreatitis. The first group of patients received standard therapy according to the 2020 clinical guidelines of the Russian Society of Surgeons. The second group received standard therapy combined with the new selective enteral nutrition method. The observation period lasted until the end of the hospital treatment, including five mandatory visits and a final visit to the physician. A follow-up survey was conducted three months or more after discharge to identify delayed episodes of acute pancreatitis complications, focusing on the incidence of infectious complications and surgical interventions.

RESULTS

RESULTS. A total of 148 patients were treated. Patients were evenly distributed by etiology, gender, age, and body mass index. The analysis revealed that the number of patients with infectious complications decreased from 19.3% in the comparison group to 6.67% in the study group (p=0.041). The number of patients requiring surgical interventions was 5.0% in the study group compared to 19.3% in the comparison group (p=0.017).

CONCLUSIONS

CONCLUSIONS. The study demonstrated that the use of the new method in the comprehensive treatment of acute pancreatitis, including early enteral feeding at the onset of the disease, may improve the course of the disease by reducing the incidence of infectious complications and the number of necessary surgical interventions.
