Лечение таких пациентов до сих пор представляет большие трудности для врача-хирурга, занимающегося этой проблемой. На реконструктивно-восстановительном этапе они заключаются в отсутствии критериев выбора способа пластики и стандартизированных оперативных технологий. Общепризнанным считается применение аутологичных тканей: большого сальника и мышечных лоскутов. В статье представлен опыт лечения пациента с постстернотомным медиастинитом. Хирургическая тактика была двухэтапной. Показана эффективность применения вакуум-ассистированных повязок. Выполнение окончательного дебридмента и подготовки раны с последующей пластикой дефекта полнослойным лоскутом большой грудной мышцы на перфорантной ветви внутренней грудной артерии привели к выздоровлению пациента.</p></abstract><trans-abstract xml:lang="en"><p>   Poststernotomy mediastinitis is the most severe and dangerous complication in cardiac surgery. Treatment of such patients still poses great difficulties for the surgeon dealing with this problem. At the reconstructive stage, they consist in the lack of criteria for choosing a method of plastic surgery and standardized surgical technologies. The use of autologous tissues is generally performed: greater omentum and muscle flaps. The article presents the experience of treating a patient with poststernotomy mediastinitis. The surgery was two-staged. The effectiveness of using vacuum-assisted dressings has been demonstrated.
