Известно, что для пациентов с тяжелым течением новой коронавирусной инфекции (COVID-19) характерно развитие COVID-19-ассоциированной дизавтономии (COVID-19-ДА). На данный момент нет исследований, посвященных изучению влияния этого феномена на течение и исходы заболевания у самой тяжелой когорты пациентов с COVID-19, а именно у пациентов с потребностью в проведении вено-венозной экстракорпоральной мембранной оксигенации (ВВ ЭКМО).</p></sec><sec><title>   ЦЕЛЬ ИССЛЕДОВАНИЯ</title><p>   ЦЕЛЬ ИССЛЕДОВАНИЯ. Изучение влияния различных фенотипов COVID-19-ДА на параметры производительности и эффективность ВВ ЭКМО, показатели газообмена и гемодинамики у пациентов с COVID-19.</p></sec><sec><title>   МАТЕРИАЛ И МЕТОДЫ</title><p>   МАТЕРИАЛ И МЕТОДЫ. В исследование вошли 20 пациентов — 12 женщин (60 %) и 8 мужчин (40 %) с COVID-19, которым проводили ВВ ЭКМО и суточное холтеровское мониторирование с оценкой спектральных параметров вариабельности сердечного ритма (ВСР): низкочастотного компонента записи (LF), высокочастотного компонента записи (HF), отношения низкочастотного компонента записи к высокочастотному (LF/HF) на 1-е, 3-и и 5-е сутки проведения ВВ ЭКМО. На основании выявленных фенотипов COVID-19-ДА пациенты были разделены на три группы. Был проведен статистический анализ групп по параметрам газообмена, гемодинамики и производительности ВВ ЭКМО.</p></sec><sec><title>   РЕЗУЛЬТАТЫ</title><p>   РЕЗУЛЬТАТЫ. Уровень парциального давления углекислого газа в артериальной крови (paCO2) при фенотипе с пониженным тонусом симпатического и высоким тонусом парасимпатического отдела вегетативной нервной системы — ВНС (пСвП) был статистически значимо выше, чем при фенотипе с нормальным тонусом симпатического и высоким тонусом парасимпатического отдела ВНС (нСвП) при равных параметрах производительности ВВ ЭКМО. Частота сердечных сокращений (ЧСС) при фенотипе нСвП была статистически значимо ниже, чем при фенотипе пСвП. Также при фенотипе пСвП было выявлено статистически значимое нарастание дыхательной дисфункции в динамике. Отлучение от ВВ ЭКМО при фенотипе нСвП было успешно в 50 % случаев, тогда как при фенотипе пСвП отлучение от ВВ ЭКМО наблюдалось у 7,2 % пациентов. Статистически значимых отличий впоказателе летальности получено не было. Наиболее частой причиной смерти пациентов обеих групп был септический шок.</p></sec><sec><title>   ВЫВОДЫ</title><p>   ВЫВОДЫ. Фенотип COVID-19-ДА, проявляющийся пониженным тонусом симпатического отдела и повышенным тонусом парасимпатического отдела вегетативной нервной системы, приводит к низкой эффективности проводимой вено-венозной экстракорпоральной мембранной оксигенации, результатом чего у таких пациентов является ее статистически значимо более редкое прекращение.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   RELEVANCE</title><p>   RELEVANCE. It is known that patients with severe cases of the novel coronavirus infection (COVID-19) are characterized by the development of COVID-19-associated dysautonomia (COVID-19-DA). At the moment, there are no studies examining the impact of this phenomenon on the course and outcomes of the disease in the most severe cohort of patients with COVID-19, namely those requiring venovenous extracorporeal membrane oxygenation (VV ECMO).</p></sec><sec><title>  AIM OF STUDY</title><p>  AIM OF STUDY. The purpose of the presented work is to study the effect of different COVID-19-DA phenotypes on the performance parameters and effectiveness of VV ECMO, gas exchange and hemodynamics in patients with COVID-19.</p></sec><sec><title>   MATERIAL AND METHODS</title><p>   MATERIAL AND METHODS. The study included 20 patients, 12 (60%) women, 8 (40%) men, with COVID-19 who underwent VV ECMO. Patients underwent 24-hour Holter monitoring with assessment of the spectral parameters of heart rate variability (HRV): the low-frequency component (LF), the high-frequency component (HF), the ratio of the low-frequency component to the high frequency component (LF / HF) on days 1, 3, 5 of the VV ECMO. Patients were divided into three groups based on the identified COVID-19-DA phenotypes. The groups were compared in terms of gas exchange, hemodynamics, and VV ECMO performance parameters.</p></sec><sec><title>   RESULTS</title><p>   RESULTS. The level of partial pressure of carbon dioxide in arterial blood (paCO2) in the phenotype with low sympathetic tone and high tone of the parasympathetic division of the autonomic nervous system (ANS) (lShP) was significantly higher than in the phenotype with normal sympathetic tone and high tone of the parasympathetic division of the ANS (nShP) with equal VV ECMO performance parameters. The heart rate (HR) in the nShP phenotype was significantly lower than in the lShP phenotype. A significant increase in respiratory dysfunction was revealed over time in the lShP phenotype. Weaning from VV ECMO in the nShP phenotype was successful in 50 %, whereas in the lShP phenotype, weaning from VV ECMO was observed in 7,2 % of patients. No significant differences in the mortality rate were obtained.
