Представлено применение реолитической тромбэктомии (РТ) у 2 пациентов с массивной тромбоэмболией легочной артерии (ТЭЛА). В первом наблюдении реолитическая тромбэктомия была выполнена в качестве второго этапа лечения после системной тромболитической терапии. В случае со вторым больным от тромболитической терапии решено было отказаться в связи с высоким риском кровотечения из острой язвы луковицы двенадцатиперстной кишки (ДПК). В раннем послеоперационном периоде у пациента с большим объемом тромбэктомии (500 мл) развилось осложнение — гемолиз в сочетании с острым повреждением почек, которое потребовало проведения сеансов плазмафереза и продленной вено-венозной гемофильтрации. Применение РТ сопровождалось хорошим клиническим и ангиографическим результатом у обоих больных. </p></abstract><trans-abstract xml:lang="en"><p>Abstract. We report the use of rheolytic thombectomy in two patients with severe pulmonary embolism. In first case RT was performed as the second step in treatment of pulmonary embolism after systemic thrombolytic therapy. In second case systemic thrombolytic therapy was not performed because of extremely high risk of duodenal ulcer bleeding. Hemolysis and acute kidney injure with requirement of plasmapheresis and continuous venovenous hemofiltration was required in patient who received big volume of thrombectomy (500 ml).
